The training will be held on Tuesday, May 30 at 6:30 p.m. and Wednesday, May 31 from 8:00 a.m. to 3:00 p.m.

Hotel Information:
Each participant is responsible for his/her own hotel accommodations. Please call the Marriott for reservations at 812-867-7999.

Group Name: Social Norms Training

Group Rate: The group rate is $70 plus tax per night. The hotel has blocked rooms to accommodate the evening of May 30. If you have special lodging needs, please mention those requests when you call in to make reservations.

NOTE: Deadline for making hotel reservations at the group rate is Friday, May 26, 2006

Check-in/check-out times: Hotel check-in available at 3:00 p.m. Check-out is 11:00 a.m.

Guarantee Reservations: Upon check-in, present a valid credit card. The hotel will require a deposit equal to the room rate and tax for each reservation. An individual’s deposit is refundable to that individual if the hotel receives notice of an individual’s cancellation by 6:00 p.m. date of arrival. NOTE: The hotel will provide a written confirmation number upon request.

Participants requiring special arrangements (i.e., large print, sign interpreters, etc.) should contact Jan Caron at 800-782-1878 or email her at jcaron@miph.org. Requests must be submitted by May 22, 2006, for us to accommodate those needs.
